Pittsburgh’s kindergartners fall below the measles safety threshold, testing the city’s vaccine legacy

Since August 2026, Pittsburgh and surrounding areas have faced a significant measles outbreak, dominating local news. The Pennsylvania Department of Health reported 693 measles cases across 38 of the state’s 67 counties as of September 14, 2026, including 133 hospitalizations and four deaths, all among unvaccinated residents. This marks a sharp increase from just 16 cases statewide in 2025. The outbreak began with a potential exposure alert at UPMC Children’s Hospital in Allegheny County, where two confirmed cases were diagnosed on August 27, 2026. The situation has prompted health officials to urge residents to check their vaccination status to prevent further spread.
Pittsburgh has a long history in vaccine development, notably as the birthplace of the polio vaccine. In the 1950s, Dr. Jonas Salk developed the first polio vaccine at the University of Pittsburgh, and children in the area, including the author’s father, participated in the vaccine trials. Dr. Thomas Francis, who coordinated the polio vaccine field trial involving 1.8 million children, declared the vaccine “safe, effective, potent.” This legacy underscores the city’s role in combating vaccine-preventable diseases, making the current measles outbreak a test of whether this history still influences local vaccination rates today.
Measles is an ancient and highly contagious respiratory virus that has existed for thousands of years, with archaeological evidence dating back to 3000 B.C.E. in the Tigris and Euphrates river valley. It spreads easily through the air, and infected individuals can transmit the virus 3 to 5 days before symptoms appear, a process known as asymptomatic spread. Symptoms include a high fever (up to 105°F), cough, runny nose, conjunctivitis (eye inflammation), and Koplik spots (small white or bluish-white spots inside the cheeks), followed by a distinctive red rash. Before vaccination, measles infected up to 3 million people annually in the U.S., leading to 48,000 hospitalizations and 500 deaths. The disease was eliminated in the U.S. in 2000 through vaccination but has resurged due to declining vaccination rates.
To prevent measles from spreading, at least 95% of the population must be vaccinated with the MMR vaccine (measles, mumps, and rubella). The MMR vaccine is typically given in two doses to children aged 4 to 6. In Allegheny County, the kindergarten MMR vaccination rate was 91.8% for the 2025-26 school year, below the 95% threshold needed to stop transmission. This decline has been tracked for over a decade. Pennsylvania’s current outbreak, with four times as many cases in 2026 as in the previous 30 years combined, highlights how even small drops in vaccination coverage can lead to large outbreaks. The MMR vaccine is free for uninsured, underinsured, or Medicaid-insured children at Pennsylvania’s 59 state health centers.
The risk of developing measles after exposure depends on vaccination status. Vaccinated individuals or those who have had measles have about a 3% chance of infection and do not need to quarantine, though they should monitor for symptoms. Unvaccinated individuals, including infants, face a 90% risk of infection and must quarantine for 21 days to prevent asymptomatic spread. If symptoms develop during quarantine, isolation for at least 14 days is required. Measles is serious: about 20% of cases lead to pneumonia, many require hospitalization, and a few result in death. Each measles case costs the healthcare system approximately $43,000. The virus can also weaken the immune system, leaving survivors vulnerable to other infections for months or years.
Vaccination against measles not only protects individuals but also creates *community immunity*, where transmission declines as more people are vaccinated. To achieve this, at least 95% of the population must be immunized. Health officials recommend checking vaccination status, especially for those exposed to measles. The MMR vaccine is widely available at no cost for eligible children in Pennsylvania. The current outbreak in Pittsburgh serves as a reminder of the city’s historical role in vaccine development and tests whether this legacy will drive higher vaccination rates today to prevent further spread.
